The Case Management Coordinator (CMC) is a motivated and experienced individual working within
a civil society organization or faith-based organization who is responsible for managing
the flow of data and providing technical and technical support. supervision of
social workers. For the Consolidation of Systems and Services for the Management of Orphans and
Vulnerable Children in Cameroon (CoSMO) project, the case management coordinator is typically
on staff of the sub-recipient’s implementing partner. The project is implemented
by the National Episcopal Conference of Cameroon (NECC) with funding from the United States Agency
for International Development (USAID).
To strengthen the resilience of families infected or affected by HIV, CoSMO’s strategy centers
on a case management approach in which families and OVC receive services
based on an individualized needs assessment and the development of plans for recovery. case. Case plans
are implemented through the provision of tiered services, closely monitored, and the purpose of the
case plan is evaluated to prepare for discharge of a family that has achieved resilience (i.e. -meaning the achievement of the
case plan).

 

 

 

The Case Management Coordinator will be responsible for overseeing and providing
technical support and supervision of social workers in the case management process. Additionally, the
CMC will supervise the activities of youth mentors during safe space activities, collaborate with
liaisons to supervise the activities of adolescent champions when facilitating
community support groups, including coordinating direct services and indirect resources provided
to project participants. . The CMC will work closely with the MEAL Coordinator and
SR-IP data entry staff to ensure that services provided to
project beneficiaries are coordinated. In collaboration with the HIV liaison, facilitate bidirectional referral
and counter-referral of CLHIV, HIV-positive pregnant and lactating adolescent mothers,
HIV-exposed infants at risk of treatment interruption between health facilities and
between the SR-IP and social centers.
Specific Responsibilities:
A.
 Coordinate the activities of stakeholders;
 Develop individualized case plans and ensure their implementation;
 Participate in the case management process, including graduation;
 Ensure that the organization joins the targeted cohort on a quarterly basis;
 Ensure effective link and referrals to health establishments,
social centers, legal services, etc.

 

 Collaborate with the MEAL Coordinator and Project Coordinator to prepare and submit
timely monthly activity/narrative reports to the CENC;
 Work closely with the Project Coordinator to schedule meetings with
the NECC Zonal Team for mentoring, supervision and training;
 Prepare and timely submit monthly activity/narrative reports accompanied
by data, in collaboration with the MEAL Coordinator, Data entry clerk and the
project coordinator for subsequent submission to CENC;
 Work closely with the project coordinator to schedule meetings
with the CENC team for mentoring, supervision and trainings.
B. Family File Management, File Closure and Project Closure
 Ensure that project participant files are properly stored in a
secure cabinet in the office. He/she is responsible for managing the confidentiality of
participant files;
 Track and document all graduation, transfer and attrition cases. Ensure that
this exit route is always included in the monthly narrative report to better
calculate the OVC_SERV indicator;
 Implement the transition phase of project closure.
C. Supervision, Monitoring and Staff Development
 Provide monthly supervision and support to social workers, field workers
/PSPs and adult mentors including regular feedback to
social workers;
 Carry out monthly monitoring of case management activities in the field (evaluation and
re-evaluation of documents, and production of case plans);
 Monitor the performance of social workers in the organization;
 Ensure that the organization achieves performance on a quarterly basis.
D. Management of quality control and assurance
 Organize a weekly meeting on data collection and quality in collaboration
with the MEAL coordinator and the Data entry Clerk;
 Ensure that CENC staff complete the quality assessment register during
field monitoring visits and ensure the implementation of recommendations;
 Carry out home visits every month and quarter to ensure that beneficiaries
receive the declared care;
 Supervise the implementation of SIMS corrective action plans;
 Ensure that CoSMO standard operating procedures are constantly used
to ensure the quality of services provided by social workers;
 Ensure the continuous strengthening of the technical capacities of social workers.
Qualifications:
 Bachelor’s degree in social sciences, health sciences or project management,
Higher national nursing diploma;
 Have at least 03 years of experience in development activities, including at least 01 or
02 years in project management;
 Experience with OVC care and support programs;
 Demonstrated leadership ability with strong interpersonal skills;

 Strong verbal and written skills in English or French. A working knowledge
of English or French is required.
 Strong IT skills (Word, Excel, Microsoft Teams,
Skype/Zoom/Google meetings);
 Experience or proven ability to work collaboratively with local authorities, close links
with other organizations (PEPFAR clinical partners, MINSANTE institutions.)
Collaborative relationship:
Internal: MEAL Coordinator, Accountant, Liaison Officer, Field Agents/ PSPS, Youth mentors,
adolescent champions, local authorities, OVC, project beneficiaries.

COMPOSITION OF FILES
– A handwritten, unstamped request addressed to the Secretary General of CODAS/Caritas;
– A cover letter ;
– An updated detailed Curriculum Vitae (CV);
– A copy of the highest diploma;
– A photocopy of the National Identity Card;
– Photocopies of any other document (diploma, certificate, work certificate,
recommendations from previous employers if possible) attesting to their experience.

DEADLINE FOR RECEIVING FILES
Complete files are expected at the CODAS-Caritas secretariat in N’Gaoundéré, no later than
October 27, 2023 at 3 p.m.
CANDIDATES SELECTION PROCEDURE
After study of the application files by the recruitment committee, only the
preselected candidates will be contacted for the rest of the process. The selection process itself will
take place in two phases:
1-The written phase
The preselected candidates will take the written test at the CODAS Caritas Ngaoundéré headquarters (for candidates
residing in Ngaoundéré) and in the CoSMO project representations on Monday 30 October
2023 from 9 a.m. sharp.
2-The oral test
The candidates selected at the end of the written phase will appear before a jury for an oral examination on Tuesday
October 31, 2023 from 9 a.m.
NB: The successful candidate will be invited for a final interview with the Secretary General of
Codas Caritas of Ngaoundéré to finalize the terms of their employment.
