Background and rationale

YOUTH DEVELOPMENT FOUNDATION (YDF) is a non-profit and apolitical association whose objective is to contribute to the promotion of health in Cameroon. This association under Cameroonian law was created in 1996, it is the manifestation of the will of a group of young people to empower themselves. It participates in building the capacity of communities in the area of ​​SDSR, promotes rights and information on reproductive health, as well as related rights, and conducts related advocacy. Moreover, Y DF participates in the setting up of networks of useful and effective partners.
Y DF is Sub Recipient (SR) under the CHILL PROJECT funded by USAID, whose Principal Recipient is CARE and HEALTH PROGRAM (CHIP)

THE (THE) CASE MANAGER (CM)
Under the supervision of the Case Manager Lead, the (the) Case Manager will have the following tasks:
Missions Du Poste:
• Follow and support the beneficiaries of Y DF living with HIV.
Main Tasks:
Placed under the responsibility of the lead Case Manager, the Case Manager must:
• Carry out pre and post counseling;
• Carry out follow-up counselling,
• Organize and lead discussion groups, support groups and any other similar activity for beneficiaries living with HIV •
• Provide psychological support for survivors of GBV •
• Conduct active search for beneficiaries lost sight of;
• Facilitate therapeutic education sessions and adherence support consultations;
• Write reports of activities carried out:
• Properly fill in all the collection tools (paper and digital);
• Properly maintain and update cohort registers and patient follow-up databases:
• Maintain and update records of positive beneficiaries;
• Report to his supervisor any case of fraud observed both at the DIC and during activities;
• Actively participate in all activities of the association Contribute actively to the search for positive cases.
Job Profile

 

• Bachelor’s degree (or equivalent) +2 in social sciences;
• Have a good knowledge of HIV and other related topics;
• Have at least 2 years of experience in counseling and psychosocial support for PLHIV;
• Have experience in a similar position;
• Mastery of computers ;
• Have a good knowledge of key and vulnerable populations;
Good knowledge of French;
• Knowledge of English and/or a local language of the intervention area would be an asset
• Have knowledge of index case and network strategies.
