Supervised by: ASSISTANT TO THE HEALTH NUTRITION PROGRAM MANAGER
Location: The candidate must have MORA as their place of residence
OBJECTIVE OF THE POSITION:
Ensure the referral and counter-referral of cases of acute malnutrition with complications as well as all other patients (children, pregnant and breastfeeding women) transferred to the CNTI and hospitals.
IN PARTICULAR, YOU WILL HAVE THE FOLLOWING RESPONSIBILITIES:
Mission 1: Responsible for the transfer vehicle.
Tasks :
- Plan and coordinate movements of the vehicle in charge of transfer
- Ensure the hygiene of the vehicle
- Make sure you have the necessary equipment present and functional
Mission 2: Ensure compliance with the national protocol for the management of acute malnutrition, for all beneficiaries referred or counter-referred by the CNTI and the PCIME Protocol for all children referred to pediatrics.
Tasks :
- Master the protocols for the management of acute malnutrition according to the national policy in force
- Master PCIME flowcharts
- Master the treatment of complications associated with malnutrition
- At the time of admission and in collaboration with the CNAS/CNAM team, identify the patients most at risk who must be transferred to the CNTI if the state of health requires it and those who do not respond to treatment
- Ensure that the criteria for admission and transfer of beneficiaries are respected and that the reason for transfer is justified:
- Prepare and verify the files of referred and counter-referred patients
- During the transfer, ensure first aid and take the first dose of treatment,
- Be able to document each referral/counter-referral case (discharged cured, death, return to the community)
- Write activity reports on these movements.
Mission 3: Ensure the well-being of patients and their companions
Tasks :
- Ensure that the patient will always be accompanied by a family member (only one companion / patient)
- Explain to the accompanying person and make sure they understand their transfer (Stay at the CNTI)
- Carry out regular follow-up for beneficiaries during their stay and when they leave the CNTI.
- Ensure the transfer of patients in good travel conditions
Mission 4: Manage the emergency trunk
Tasks :
- Establish an emergency trunk to put in the vehicle
- Prepare orders in advance so as to never be out of stock;
- Replenishment of his emergency trunk each evening
- Write a drug consumption report,
- Writes a monthly activity report (number of transfers, number of counter-transfers, morbidity, etc.)
Mission 5: Ensure the reference and counter-reference between the different structures
Tasks :
- Provide the necessary care to guarantee safety during transport before departure (insertion of infusion, vascular replacement, treatment of hyperthermia, management of pain, administration of sugar water) if necessary and according to the protocol.
- Ensure that the transfer voucher is correctly completed.
- Complete the transfer register
- Install the patient as comfortably as possible
- Provide appropriate care during transport and ensure medical supervision throughout the journey.
- Make clear and detailed transmissions to the hospital staff receiving the patient.
- Responsible for monitoring the patient during his hospitalization with the visit of the patient every evening to follow his evolution
- Organize his return according to the discharge date authorized by the doctor from the hospital to the health center of origin
Mission 6: At the request of the Program Manager, support field teams in care activities and participate in decision-making for children to be transferred
Tasks :
- Identify the patients most at risk on admission, direct patients to the room in order to receive first aid;
- Identify patients who need to be transferred to the CNTI or hospital and those who do not respond to treatment;
- In collaboration with the CCs, participate in decision-making for most children to be referred to the CNTI and to the hospital.
- At the time of discharge to the CNTI and the hospital: check that the discharge criteria are met, that the immunization is satisfactory;
SPECIFICATIONS
This job profile may be subject to permanent modifications according to the evolution of activities. These modifications will be defined and discussed between the employee and Action contre la Faim.

DIPLOMAS/LEVEL OF STUDY/EXPERIENCE:
- State Diploma of Nursing
REQUIRED SKILLS :
- Patience, good interpersonal skills, good ability to work in a team, good management of priorities, precision and rigor in his work and the monitoring of protocols, good writing skills, Ability to formulate proposals and take initiatives, creativity-Availability. Linguistic skills (French, Folfode, Arabic…)
