Missions and Responsibilities
General Objective
To provide appropriate case management in a timely manner by assessing the needs of people with protection problems, by developing and implementing tailor-made care plans based on needs and ensuring appropriate follow-up while creating a climate of trust and care.
Responsibilities
• Conduct intake interviews with people identified by the team during outreach or referred by other agencies, open a case if vulnerability criteria are met
• Perform comprehensive assessments for each client after obtaining consent and consider emotional, behavioral and social factors that affect their psychosocial well-being, as well as protective and risk factors
• Conduct home visits to assess clients’ living conditions when necessary
• Develop a plan tailor-made support with the client, based on the needs identified during the assessment
• Refer clients to appropriate services and, if necessary, accompany them to different service providers
• Provide ongoing basic emotional support and counseling during regular follow-up meetings, building a relationship of trust based on confidentiality protocols
• Document cases on the internal case management platform, ensure that any progress in the case plan and any significant changes in the client’s psychosocial well-being and general situation are timely and duly recorded
• Ensure proper and secure filing and archiving of case management files to preserve data confidentiality
• Close cases when case plan objectives are met and no further needs are identified

Requirements
Education • University degree in social sciences or related field.
Experience • 1 year of relevant professional experience
Language ▪
▪ Proficiency in French (written and spoken).
Proficiency in the local languages of Logone and Chari
