The Cameroonian Association for Social Marketing (ACMS) is seeking candidates for the position of Care Continuity Specialist (M/F) to oversee the implementation, expansion, and optimization of HIV testing services and linkage to care, in accordance with national guidelines and best practices.
Who are we recruiting?
The Care continuity Specialist will work under the supervision of the Integrated Service Delivery Specialist of the Program with the main responsibilities of
Essential Functions:
1. Provide technical advice and supervise the implementation of differentiated service delivery models, including but not limited to community dispensing, home delivery (VAD), extended hours during the weekend for ARV dispensing and viral load sampling services, and multi-month dispensing.

2. Develop tools to closely monitor the various retention indicators health facility by health facility, APS by APS.
3. Supervise the establishment and strengthening of support groups adapted to the age and category of people living with HIV to ensure continuous access to antiretroviral treatment.
4. Support the creation and operation of support groups for PvHlH by working closely with the various specialists and creating a platform for shared learning and support.
5. Adapt DSD models to the profiles of stable patients and train staff in their effective implementation. Ensure that FOSA-based and community-based DSD models are adapted to the needs of all FOSA supported.
6. Conduct targeted training and coaching sessions for psychosocial agents (PSAs) on case management, ensuring they are equipped with the necessary skills and knowledge.
7. Oversee the implementation of the comprehensive case management model in supported health facilities, focusing on improving case management practices to improve patient outcomes.
8. Support the development and maintenance of a patient tracking system, incorporating various methods such as phone calls, home visits, and DAMA protocols, to improve patient retention and follow-up.
9. Organize monthly retention meetings in all supported health facilities to discuss challenges and strategies to maintain patient engagement in care.
10. Expand the implementation of triage stations to all major entry points for PLHIV on ART, ensuring that patients receive appropriate and timely care.
11. Support quarterly joint mentoring sessions with Regional Technical Groups (RTGs) to review progress, share best practices, and address challenges in implementing continuity of care strategies.
12. Train health staff on early warning indicators of HIV drug resistance and ensure that all sites have the necessary tools (e.g., monitoring charts) to monitor these indicators.
13. Ensure the holistic management of HIV-positive people by integrating the management of associated chronic diseases (hypertension, diabetes)
14. Establish strategies to better manage men living with HIV in health facilities (Men star approach)
15. Support health facilities to systematically collect the viral load of eligible HIV-positive people by working in close collaboration with the laboratory specialist
16. Develop a tool to monitor the EAC cascade until patients are transferred to the 2nd or 3rd line
17. Ensure that all patients eligible for resistance testing have been sampled
18. Promote the message (undetectable = untransmittable) when starting antiretroviral treatment in order to generate demand for viral load tests and improve patient adherence to treatment.
19. Provide technical support for the implementation of a system hub-and-spoke referral system for the care and treatment of patients with advanced HIV disease, ensuring the streamlining of service delivery.
20. Provide ongoing mentoring to care providers on best practices for managing AHV.
21. Oversee the documentation and reporting of results of advanced disease management in supported health facilities, ensuring that services are well documented and that data are accurately entered and reported in a timely manner.
22. Oversee the training of viral load champions for peer education on HIV management and viral load monitoring, ensuring that they can effectively communicate messages to their peers.
23. Support the development and distribution of written materials in multiple languages spoken in the regions to improve health literacy and ensure that all clients have access to relevant information.
24. Facilitate the training of care providers on effective communication strategies and health literacy principles, to empower them to better engage with their clients.
25. Perform any other task related to its activities assigned to you by the hierarchy.
Qualifications and skills required:
Hold at least a State Diploma in Nursing, Master in Public Health
Have a minimum of three years of experience in the field of health care with an emphasis on continuity of care;
In-depth knowledge of HIV care and treatment protocols • Fluency in English and French.
