BACKGROUND  : In Cameroon, cervical cancer is the second most common cancer in women after breast cancer, with an age-standardized incidence rate (ASIN) estimated at 33.7/100,000 women-years in 2020, and the most common cancer in women aged 15 to 44. In 2021, Cameroon recorded 2,770 new cases of cervical cancer and 1,787 cervical cancer-related deaths, highlighting the considerable burden of the disease on the country’s health system. Reducing the relatively high burden of cervical cancer in Cameroon and aligning with the World Health Organization (WHO) global strategy to eliminate cervical cancer remains a challenge. Given this critical situation, the Cameroonian government, with the support of UNICEF and other key technical and financial partners, introduced the HPV vaccine into the Expanded Immunization Program (EPI) for girls in October 2020. aged 9 years.
When the HPV vaccine was introduced in Cameroon, the target population was estimated at 340,000 adolescent girls, of whom approximately 90% were in school. Therefore, the vaccine was introduced using a health facility and school-based approach, with the aim of achieving 70% coverage in the first year. But faced with the low acceptance of the vaccine, this objective was reduced to 25% in the first year, 40% in the second year and 60% in the third year. However, the school-based strategy did not produce the expected results. Vaccination coverage in April 2021, a few months after introduction, was 17% (HPV1), below the objective, with the lowest coverage recorded in the 3 Regions: Center (2%), Littoral (1 %) and West (4%). At the end of 2022, HPV vaccination coverage was 20.3% for the first dose (HPV1) and 5.9% for the second dose (HPV2), highlighting the low rate of use and challenges related to acceptance of this vaccine.
Reasons for the poor performance of HPV vaccination in Cameroon include the introduction of the HPV vaccine during the COVID-19 pandemic, against a backdrop of media misinformation campaigns and anti-vaccination rumors.

 

 

OBJECTIVES:  the mission assigned to the consultant aims to identify, describe in detail and analyze the major behavioral barriers as well as the individual and collective motivations for the demand and use of HPV vaccination services at all stages of the care pathway. .
The objectives of the consultant’s mission are as follows:

  • List and describe the usual practices of families and communities in the targeted Health Districts, relating to the vaccination of young girls in general and vaccination against HPV in particular;
  • Identify and describe barriers to demand and use of HPV vaccination services throughout the care pathway;
  • Analyze the determinants that build and maintain behavioral barriers to HPV vaccination of adolescents/girls throughout the care pathway;
  • Identify and analyze the logic of the actors/carriers who prevent as well as those who promote the demand and use of HPV vaccination services by all or part of the members of the different groups/communities in the targeted Health Districts;
  • Identify and analyze the bottlenecks (structural and situational socio-economic factors) that prevent the demand and use of HPV vaccination services by all or part of the members of the different groups/communities in the targeted Health Districts;
  • Identify and analyze individual and collective motivations at all levels of the care pathway likely to accelerate the demand for and use of HPV vaccination services by adolescents/girls;
  • Analyze useful synergies that could sustainably promote demand and use of HPV vaccination services by adolescents/girls;
  • Conduct an analysis of societal actors and the roles they can play in promoting demand and use of HPV vaccination services by adolescents/girls;
  • Propose a logical and coherent approach for promoting demand for and use of HPV vaccination services by adolescents/girls.

SPECIFIC TASKS:

Under the supervision of the SBC Section Head, the consultant will conduct robust anthropological research, using iteration and reflexivity to understand behavioral barriers and individual and collective motivations for HPV vaccine demand and use. by the girls. At a minimum, it should undertake the following tasks, which can be refined during an initial discussion with the study monitoring team:

  1. Consultations with internal technical specialists (UNICEF) and external partners and experts (local and/or international) to determine needs and expectations related to the CBS strategic planning process and its outcomes in order to conduct a stakeholder analysis .
  2.  Identify problems and produce a behavioral map that will result in a list of potential experimental interventions.

ACTIVITIES AND SPECIFIC DELIVERABLES:

The consultant anthropologist will produce high quality, evidence-based written materials that include triangulated data and analysis. Final versions must be revised and free of errors. The working language and deliverables of this project will be French and English. Below are the details of the products to be delivered:

Education : 

Hold a Master’s degree or a Bac +5 university degree in anthropology or sociology.

Experience :

  • More than 10 years of experience in conducting qualitative surveys,
  • Have carried out at least three similar missions over the last five years,
  • Experience in conducting similar studies in social anthropology
  • Mastery of qualitative data analysis tools and software (NVivo for example),
  • Good report writing skills,
  • Be able to work in difficult contexts, particularly in terms of security.

Language requirements:

  • French and English are required.

Financial Proposal  : The financial proposal must include costs (providing a per diem rate as justification) for each task, including consultant fees, travel costs, proposed per diems, communication costs and any other proposed costs.
